BASIC RESPONSIBILITIES:
This position will provide quality occupational therapy services through education and coaching to families and colleagues in order to maximize the functional independence of children birth to three years within their family's daily routines and activities. The therapist will be a member of a multidisciplinary early intervention team supporting infants and toddlers with developmental delays and their families in the child's natural environment.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
* Assist in the identification, assessment, and intervention for children referred to and/or receiving AzEIP services.
* Evaluate children referred to the Arizona Early Intervention Program using established evaluation procedures and standardized assessments.
* Conduct observations in the child's natural environment or least restrictive setting.
* Ensure that families receive accurate information in a timely manner.
* Ensure families receive accurate explanations of the child's assessment and that they understand the scores.
* Communicate clearly in understandable language and without use of unnecessary jargon.
* Develop and provide coaching for family members to promote the child's development and encourage the child's participation in home and community activities, as part of the Individualized Family Service Plan (IFSP) team.
* Participate on multidisciplinary team, acting as team lead and/or service coordinator, as appropriate.
* Provide program services effectively and in a manner that reflects understanding of the centrality of relationships in supporting the child and family growth and development.
* Develop services that address the functional needs of the child to improve the child's ability to perform tasks in home, schools, and community settings.
* Provide and accept colleague to colleague coaching and other agency programs when appropriate.
* Instruct family members in the proper and safe usage of adaptive equipment and assistive technology.
* Assist in the coordination of services and make appropriate referrals as needed.
* Participate in the determination of eligibility for the Arizona Early Intervention Program (AzEIP) and assist in the determination of eligibility for AzEIP participating agencies.
* Demonstrate practices consistent with the AzEIP Standards of Practice, which are based on state and federal legislation and best practice recommendations.
* Collaborate with community agencies and professionals in order to provide timely and seamless services to eligible children and families.
* Participate in program evaluation and monitoring requirements.
* Complete all progress reports, evaluation reports, and paperwork in a timely manner.
* Develop and maintain all client progress reports and documentation, in compliance with agency or program rules and regulations.
* Attend weekly core team meetings.
* Comply with privacy laws and requirements under HIPAA regulations.
* Follow all safety procedures as required by organization policy and procedure.
* Maintain an effective and professional working relationship with families, co-workers, and referral sources.
* Work a flexible schedule to meet the needs of families.
* Perform Service Coordination roles and responsibilities when implementing the dual role Service Coordination model (see Service Coordination Job Description for requirements).
* Function as an enthusiastic, competent, contributing member of ESBF staff.
* Participate in education and support for program staff when requested.
* Provide consultation and support to program staff and other participating programs when requested.
* Attend and participate in required staff meetings.
* Promote and follow practices that support health and safety of staff.
* Develop and maintain positive relationships with program participants, staff, collaborating people, programs and state agencies.
* Submit accurate and complete paperwork, including proof-reading for correct grammar, spelling and format.
* Maintain agreed upon work schedule, be punctual, keep appointments, and use time effectively.
* Use supervision effectively, accept feedback in a professional manner, and accept responsibility for job performance.
* Maintain professional certification/licensure as required.
* Attend required professional development opportunities and maintain knowledge of current research and effective approaches.
* Communicate essential information with ESBF team staff and team members accurately and in a timely manner.
* Understand and maintain private and sensitive information within the confines of need-to-know parameters.
* Comply with State, Federal and Other Relevant Regulations.
* Follow state regulations and guidelines regarding choice and use of formal testing instruments.
* Record client treatments, responses and progress as required by the program and AzEIP contract requirements.
* Complete all progress, transition, and exit reports in a timely manner.
* Participate in documentation reviews, when requested.
* Attend all required meetings.
* Attend all required training.
* Comply with privacy laws and requirements under HIPAA regulations and understand the consequences of non-compliance.
* Follow all safety procedures as required by organization policy and procedure and understand the consequences of non-compliance.
* Bring issues of concern to designated supervisor in a timely manner.
* Report suspected abuse to Child Protective Services and supervisor, complete required documentation.

QUALIFICATIONS:
* Licensed as an Occupational Therapist in the state of Arizona
* The Arizona Early Intervention Program's Standards of Practice must be satisfied through course work and/or testing option within the first three years of employment
* Knowledge of best pediatric occupational therapy practices
* Current certification in Infant/Child CPR and First Aid
* Current valid driver's license, proof of insurance and driving record that meets the standards of the agency
* Current proof of negative TB test
* Interact with participants, families and staff in a positive manner
* Familiarity with disabilities and developmental issues for children birth to three years
* Knowledge of developmentally appropriate practices
* Knowledge of reflective supervision
* Knowledge of cultural differences and adaptation of planned programming based on such knowledge
* Communicate effectively and professionally verbally and in writing
* Maintain and enforce confidentiality
* Comprehension of Easter Seals Blake Foundation's operating policies and procedures
* Bilingual in Spanish and English preferred
